Why do health premiums change based on your city?

According to the Swiss compulsory health insurance law (LAMal/KVG), premiums are not calculated uniformly for the entire country, but are strictly based on the actual health care costs generated within each canton. Additionally, most cantons are internally subdivided into "Premium Regions" (Prämienregionen) delimited by Postal Code. Therefore, your official place of residence defines your premium. Why does my health insurance cost differently than in another city in my same canton?
In Switzerland, the LAMal law divides cantons into up to 3 "Premium Regions" (Prämienregionen). The price depends on the statistical health care costs of your exact postal code. That is why moving to the next town can drastically alter your monthly rate.
If I move to another canton, must I change my health insurer (Krankenkasse)?
It is not mandatory, but it is strategically highly recommended. Your current insurer will adjust your fee to the official rate of your new city. The company that was the cheapest in your old canton is rarely still the most economical option in your new destination.
Do basic medical benefits (Grundversicherung) change by canton?

Each canton has its own rules, income limits, and deadlines for requesting these subsidies (IPV). If you move to a new canton, you must find out the details and register again at the corresponding cantonal office of your new residence.
Are prices updated automatically when moving?
Yes. Your current insurer will adjust your rate to the new region from the month following the moment you officially notify your change of address (Abmeldung/Anmeldung) at your new municipality (Gemeinde).
